HOMECARE/HOSPICE REGISTERED NURSE

Black River Memorial Hospital has a benefited, part-time position available, working 64 hours per pay period as a Homecare/Hospice Registered Nurse.  This nursing position is primarily day shifts and every 4th weekend that will include an on-call rotation for weekday nights.  This position is responsible for planning and providing comprehensive patient care.  It also manages available nursing resources and collaborates with other members of the healthcare team to provide quality cost-effective patient care.

Essential Duties:

  • Conducts the initial assessment and knowledge of eligibility requirements for admission to the Hospice/Homecare program.
  • Develops an individualized plan of care that respects the patient, family and/or significant other’s wishes and needs
  • Obtains appropriate medical orders in accordance with the Hospice/Homecare plan of care.
  • Communicates regularly with physician and team members providing information and documentation regarding the physical, psychosocial and spiritual status of the patient and family. Coaches and/or mentors others in how to communicate clearly and concisely; always communicates changes in patient status utilizing the SBAR format.

Educational Qualifications:

  • Bachelor of Science or Associates degree in Nursing. 
  • Minimum of 1-year nursing experience. 
  • Experience with Hospice and/or Homecare preferred. 
  • Must have valid WI driver's license, proof of insurability and reliable transportation.
  • BLS within 6-months of employment.
  • Current Wisconsin RN License.

LABORATORY UNIT COORDINATOR

Black River Memorial Hospital is seeking a full-time Laboratory Unit Coordinator that would also serve as a Phlebotomy Tech and Clerk.  The qualified candidate must be able to perform blood draws and collect and process specimens.  Phlebotomy experience is preferred.  Clerical and secretarial experience is required along with proficiency in Microsoft Office (Word and Excel).  This benefited position is day shift, Monday through Friday, 80 hours per pay period. 

Essential Duties:

  • Collects blood specimens for a variety of laboratory tests as different collection sites for daily “stat,” timed and routine test orders for accurate and timely specimen procurement and handling.
  • Assists with ordering, receiving, or logging of specimens; follows up on unavailable specimens; relays patient laboratory test information and manages data through appropriate computer program
  • Maintains phlebotomy trays, work stations and equipment in a sanitary condition and replenishes and updates supplies.
  • Purchases supplies for the department, answers phones and responds requests.

Education Qualifications:

  • High School Diploma or General Education Degree (GED).
  • Certificate from a vocational or technical college in phlebotomy, experience as a Lab Unit Clerk or equivalent education and experience.
  • Proficient skills in operation of standard office equipment such as multi line phones, computers, fax machine, copy machine and printers.
  • Advanced knowledge in Microsoft Word, Excel, Access, Outlook, Micro Soft Office and database.
  • BLS certification within 1- year of hire date.

MAINTENANCE MECHANIC

Don’t miss this opportunity to join our Environmental Services team!  Black River Memorial Hospital has an immediate opening for an Environmental Services Maintenance Mechanic I.  The Maintenance Mechanic uses mechanical, technical, physical and communication skills in order to maintain a safe, reliable and comfortable healthcare environment.  This is a full-time, 80 hour per pay period, day shift, fully benefited position which includes on call every fifth week, Monday through Sunday. 

Essential Duties Include:

  • Performs and/or arranges for corrective and preventive maintenance as needed in the disciplines of electrical, HVAC, communications, boiler/steam, plumbing, hospital equipment and building and grounds.
  • Reads and understands blueprints as an essential part of carrying out work in various disciplines of this position.
  • Responds within 30 minutes to trouble calls when scheduled as the on-call person and addresses with problems upon arrival.
  • Uses safe methods when handling chemicals and cleaning solutions in daily work practices.

Educational Qualifications Include:

  • Associate’s degree in a maintenance field or equivalent from a two-year college or technical school.
  • Two years’ experience in maintenance related field.
  • Experience with the operation and maintenance of high-pressure boilers and auxiliary equipment.
  • Experience in reading and interpreting mechanical and electrical drawings.

NUTRITION SERVICES ASSISTANT

Black River Memorial Hospital is seeking a part-time Nutrition Services Assistant. This position is benefited, working 64 hours per pay period, hours varying between days and afternoons.  The Nutrition Services Assistant assists in the preparation, cooking and serving of food.  This position requires a high school diploma or a general education degree and basic reading and comprehension skills.

Essential Duties:

  • Assists in preparing and portioning food for patients using room service or scheduled meals, contracted food services, employees and hospital special functions following established policies and procedures.
  • Accurately and appropriately transports scheduled patient meal cart to the patient floor and delivers patient’s room service trays or late trays directly to the patient’s room.  Collects soiled trays from patient rooms and document amount of food consumed by the patient.
  • Performs various cleaning duties per department protocol.  Keeps work area clean and in a sanitary condition at all times.
  • Follows safe food handling practices.

PATIENT CARE TECHNICIAN

  • Recognizes changes in signs and symptoms and promptly reports patient’s condition to the RN.

PHYSICAL THERAPIST

Black River Memorial Hospital is seeking a full-time benefited Physical Therapist.  This position is 80 hours per pay period.  Hours are divided between the Homecare-Hospice Department (40 hours) and Inpatient Departments (40 hours).  Hours for this position are Monday through Friday, with hospital weekend rotation.  This position plans, evaluates, and administers physical therapy treatment to patients within the scope of their licensure. 

Essential Duties for this position:

  • Conducts physical therapy evaluations.
  • Interprets findings of the evaluation.
  • Develops prognosis and plan of care.
  • Provides supervision of physical therapy assistant or support staff in Homecare/Hospice.
  • Provides supervision of assistants and rehabilitation department support staff.
  • Accurately bills for services rendered.
  • Conducts discharge planning.

Educational Qualifications:

  • Bachelor of Science (B.S.) degree from an accredited Physical Therapy program.
  • Current Wisconsin license as a physical therapist.
  • Valid Wisconsin Drivers’ License and proof of insurability required.
  • At least one year of experience as a practicing Physical Therapist.

REGISTRATION CLERK

Black River Memorial Hospital is seeking a highly motivated individual to fill the full-time position of Registration Clerk.  This 72 hour per pay period position is on a rotating schedule of days and p.m.’s.  The Registration Clerk is responsible for interviewing incoming patients or representatives and accurately entering information required for admissions into the computer.   In addition, the Registration Clerk answers facility incoming telephone calls and directs them to appropriate personnel or department.

Essential Duties:

  • Greets, assists and directs all people on arrival to the hospital.
  • Registers all patients according to hospital policy.
  • Efficiently and appropriately utilizes communication/office equipment.
  • Completes pre-registration process (monitors Scheduler Books).

Educational Qualifications:

  • High School Diploma or General Education Degree (GED).
  • Six-months related training or experience.
  • Proficiently uses computer, calculator and various types of communication equipment.
  • Excellent oral and written communication skills.

  • Current licensed Wisconsin RN.
