Black River Memorial Hospital has a part-time opportunity for a Nurse Practitioner (NP) in our Homecare/Hospice Department.  This benefited position is 40 hours per pay period, day shift, fluctuating between Monday through Friday based on census and NP’s preference.   

This position evaluates and treats patients of all ages with severe chronic disease and/or end of life prognosis. In addition, this role completes patient assessments, orders and interprets diagnostic tests and completes or prescribes treatments under the supervising Hospice/Palliative Care Physician.

Essential Duties:

  • Assesses patients of all ages presenting for health care and obtains a personal medical history, appropriate physical examination and completes documentation of same using the hospital’s clinical charting system. Documents or dictates articulate and concise patient care information for the physician and medical record.
  • Performs orders and interprets diagnostic test and patient data to determine appropriate diagnostic and therapeutic procedures.
  • Instructs and counsels patients and families related to physical/mental health, self-care, medication use and management of symptoms.
  • Works collaboratively with the Homecare/Hospice and Palliative Care Director and other team members to ensure smooth day-to-day operations within the teams. Ensures the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resources utilization.

Educational Qualifications:

  • Graduate of a nationally accredited Nurse Practitioner educational program.
  • Current, valid, unrestricted Nurse Practitioner license from the State of Wisconsin; current board certification or current board certification from ANCC, ANA, or AANP for NP must be achieved within 18 months of hire; and current unrestricted DEA.
  • BLS required
  • Minimum of three years clinical experience required.  Chronic Disease Management, Palliative Care, and/or Hospice experience preferred.
  • Achieves palliative care board certification within five years of employment as Palliative Nurse Practitioner or the applicant must be functioning as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) with hospice and palliative advanced nursing practice experience of 500 hours in the most recent 12 months or 1000 in the most recent 24 months.

Black River Memorial Hospital has an immediate opening for a Patient Care Coordinator in our Lab Department.  This is a full time, benefited position, working 80 hours per pay period.  The Patient Care Coordinator assists with responsibility for planning, organization, and delivery of laboratory services.  This position is administratively responsible for the coordination, direction, implementation, and evaluation of assigned service areas and provides oversight and leadership for staff to maintain the efficient and effective function of the Lab Department.

Essential Duties:

  • Assesses plans, directs, coordinates and evaluates the delivery of care in collaboration with doctors and advanced practice practitioners to maintain standards of care in the clinical setting.
  • Assists with providing feedback to staff regarding their performance under the supervision of the Lab Director.
  • Monitors productivity on a daily basis and makes staffing adjustments. Coordinates operational activities of the Lab.  Conducts orientation and serves as a preceptor.
  • Assists with training and development regarding the latest developments in Laboratory services and changes in hospital policies/procedures/protocols.

 Education Qualifications:

  • B.S. Degree in Medical Technology.  Laboratory training culminating in national certification: MT (ASCP), MLS (ASCP).
  • Prefer additional management training or a minimum of 5 years of practical experience as a Medical Technologist with some supervisory responsibility.
  • BLS certification within 1 year of hire date.

Just graduating from Medical Laboratory Technician/Clinical Laboratory Science School?  Looking for employment that values your hard work?  Look no further!  Black River Memorial Hospital is eagerly accepting applications for an immediate, full-time opening, in our busy Lab Department at our Critical Access Hospital.   This benefited Medical Laboratory Technician (MLT) or Medical Technologist (MT) opening is 80 hours per pay period, afternoon shift, Monday through Friday with a rotating weekend and holiday schedule.   

The MT/MLT collects and processes specimens to obtain data for diagnosis and treatment of disease and accurate and timely reporting of test results.  In addition, it oversees a variety of technical activities and individual section responsibilities to ensure efficient operation of the department.

The Nutrition Services Assistant assists in the preparation, cooking and serving of food.  This position requires a high school diploma or a general education degree and basic reading and comprehension skills.   This is a part-time position working 64 hours per pay period which includes a scheduled every other weekend and holiday shift.

Essential Duties:

  • Assists in preparing and portioning food for patients using room service or scheduled meals, contracted food services, employees and hospital special functions following established policies and procedures.
  • Accurately and appropriately transports scheduled patient meal cart to the patient floor and delivers patient’s room service trays or late trays directly to the patient’s room.  Collects soiled trays from patient rooms and document the amount of food consumed by the patient.
  • Performs various cleaning duties per department protocol.  Keeps work area clean and in a sanitary condition at all times.
  • Follows safe food handling practices.

Black River Memorial Hospital is seeking a part-time Occupational Therapist in our Rehabilitation Department.  This benefited position will work Monday, Wednesday and Fridays 6-8 hours per day, totaling 40 hours per pay period.   This position plans, evaluates, directs and administers occupational therapy treatment for patients within the scope of their licensure.  The position offers the ability to work with diverse populations including hand injuries, pediatric populations, and employer-based community programs.

Essential Duties:

  • Conducts occupational therapy evaluations.
  • Interprets findings of evaluations.
  • Develops prognosis and plan of care.
  • Documents according to accepted standards.

Educational Qualifications:

  • Bachelor of Science (B.S.) or Bachelor of Arts (B.A.) degree from an accredited Occupational Therapy program.
  • Valid Wisconsin Drivers’ License and proof of insurability required.
