ADVANCED PRACTICE PRACTITIONER (PA/NP)

Black River Memorial Hospital is an independent critical access hospital serving Western Wisconsin.  Our new construction includes a $14 – million project to better serve our patients that is scheduled for completion in 2020.   This Physician Assistant/Nurse Practitioner position is 11 shifts per month, 12-hour shifts.  4-5 shifts are worked in the Emergency Department and 6-7 shifts are worked as a Hospitalist.    The Physician Assistant/Nurse Practitioner shall provide the very best high quality, cost-effective care to the patients assigned to the service through approved referral sources.

PSYCHIATRIST

Black River Memorial Hospital (BRMH) has an opening for a Psychiatrist for our new Clinic opening July 2020.  This benefited position is Monday through Friday, day shift.  This position provides direct psychiatric patient care and consultation to the collaborative care team through direct patient care and regular consultations.  The psychiatrist treats patients directly, advises the prescribing medical provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions.

Essential Duties:

  • Maintains standards of excellence in accordance with standards set forth by the Wisconsin Department of Health Services, National Association of Social Work, the Joint Commission, National Committee for Quality Assurance, and the policies and procedures of the organization.
  • Provide regularly scheduled psychiatric patient care. Evaluate, diagnose and treat patients with behavioral health disorders.
  • Receives consults from multidisciplinary team based on new patient assessment, patients who are not improving as expected under their current treatment plan, physicians orders, and multidisciplinary huddles.
  • Assesses and makes clinical evaluations of a patient’s behavioral health and diagnose illness. Incorporates a multidisciplinary approach to include the patient, clinic staff, and patient’s family/significant others to develop and implement a plan to meet the patient’s needs. Participates in multidisciplinary planning meetings and care conferences.

Educational Qualifications:

  • Doctorate’s Degree.
  • Must be licensed to practice psychiatry in the state of Wisconsin and maintain good standing on the Black River Memorial Hospital medical staff.
  • American Board of Psychiatry and Neurology Board Certification.
  • Completion of a psychiatry residency program; Prefer experience with primary care psychiatry.
  • Must maintain current BLS certification.

PHYSICAL THERAPIST

Black River Memorial Hospital (BRMH) currently has an opportunity available for a per diem Physical Therapist (PT) to join our team.  This day shift position is 12 to 24 hours per pay period, with a flexible schedule Monday through Friday, on-call Saturday working with patient’s needs.  As a PT at BRMH, you will plan, evaluate and administer physical therapy treatments to patients within the scope of your licensure.  The primary need is for inpatient acute care.  Outpatient care may be required on occasion.

Essential Duties:

  • Conducts physical therapy evaluations.
  • Interprets findings of an evaluation.
  • Develops prognosis and plan of care.
  • Documents according to accepted standards.

Educational Qualifications:

  • Minimum of Bachelor of Science (B.S.) degree from an accredited Physical Therapy program.
  • Current Wisconsin license as a physical therapist.
  • Valid Wisconsin Drivers’ License and proof of insurability required.

EMPLOYEE HEALTH NURSE

Now is your chance to join Black River Memorial Hospital’s Human Resource Department as the Employee Health Nurse.   This benefited, part-time position will work 56 hours per pay period Monday through Friday on day shift.  The Employee Health Nurse meets personnel and organizational needs by leading, planning, organizing and controlling activities regarding employee health within the Human Resources Department.  They are responsible for insuring the health of all organization employees through careful screening of potential health problems, management and follow-up of work-acquired injuries or illnesses, and assessment of the employees’ overall fitness for duty.  They also provide continuing education on Occupational Health and Safety Association topics to ensure that all personnel are adequately trained to work in the healthcare environment.

Essential Duties:

  • Develops and monitors the employee health budget under the direction of the Human Resources Director.
  • Initiates, develops, revises, reviews and updates all policies and procedures related to employee health.
  • Develops, coordinates, implements, and monitors all phases of the employee health program for the organization and its staff needed for compliance with regulatory agencies standards.
  • Administration of employee physicals (both new employees and repeat exams)
  • Employee drug screening
  • Injury management and investigation of workplace injuries or illness
  • Absenteeism/illness reporting and management
  • Exposure management
  • Administration of vaccinations (both mandatory and voluntary)
  • Employee Occupational Health and Safety Association (OSHA) education
  • Respiratory Protection Program and Hearing Protection Program compliance

Educational Qualifications:

  • Bachelor of Science or Associates degree in Nursing.
  • Minimum of 2-years’ experience as a RN in a hospital setting.
  • Current licensed Wisconsin RN.
  • Prior experience with the administration of employee health program preferred.

NUTRITION SERVICES ASSISTANT

Black River Memorial Hospital is seeking a full-time Nutrition Services Assistant. This position is benefited, working 72 hours per pay period in the afternoon.  The Nutrition Services Assistant assists in the preparation, cooking and serving of food.  A  High School diploma or general education degree (GED) is preferred and basic reading and comprehension skills.

Essential Duties:

  • Assists in preparing and portioning food for patients using room service or scheduled meals, contracted food services, employees and hospital special functions following established policies and procedures.
  • Accurately and appropriately transports scheduled patient meal carts to the patient floor and delivers patient’s room service trays or late trays directly to the patient’s room.  Collects soiled trays from patient rooms and document the amount of food consumed by the patient.
  • Performs various cleaning duties per department protocol.  Keeps work area clean and in a sanitary condition at all times.
  • Follows safe food handling practices.

Black River Memorial Hospital is seeking a self-motivated individual to fill a part-time role of Office Assistant in our Respiratory Care Department.  This Monday through Friday benefited position is 40 hours a pay period on day shift.  The Office Assistant facilitates the activities in the Respiratory Department by performing clerical duties including assisting members of the respiratory staff.

Essential Duties:

  •  Accurately performs clerical support duties as assigned: data entry, faxing, filing, use of a copy machine, payroll and preparing correspondences or forms.
  • Records, transfers and/or responds to all incoming telephone and e-mail inquiries. Processes department postal mail daily.

  • Serves as intake coordinator for new orders.  Assures that all needed documentation is obtained and completed.

  • Verifies insurance coverage for sleep studies as needed.

Educational Qualifications:

  • High School Diploma or General Education Degree (GED).
  • Associate Degree in Secretarial Science or 1-3 years’ experience in clerical or secretarial preferred.
  • Proficient use of computer, Microsoft Word, Excel and Outlook computer programs and other office equipment.
  • Ability to communicate well and work cooperatively with a team is essential.

HOMECARE/HOSPICE NURSE PRACTITIONER

Black River Memorial Hospital has a part-time opportunity for a Nurse Practitioner (NP) in our Homecare/Hospice Department.  This benefited position is 56 hours per pay period, day shift, fluctuating between Monday through Friday based on census and NP’s preference.   This position evaluates and treats patients of all ages with severe chronic disease and/or end of life prognosis. In addition, this role completes patient assessments, orders and interprets diagnostic tests and completes or prescribes treatments under the supervising Hospice/Palliative Care Physician.

Essential Duties:

  • Assesses patients of all ages presenting for health care and obtains a personal medical history, appropriate physical examination and completes documentation of same using the hospital’s clinical charting system. Documents or dictates articulate and concise patient care information for the physician and medical record.
  • Performs orders and interprets diagnostic test and patient data to determine appropriate diagnostic and therapeutic procedures.
  • Instructs and counsels patients and families related to physical/mental health, self-care, medication use and management of symptoms.
  • Works collaboratively with the Homecare/Hospice and Palliative Care Director and other team members to ensure smooth day-to-day operations within the teams. Ensures the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resources utilization.

Educational Qualifications:

  • Graduate of a nationally accredited Nurse Practitioner educational program.
  • Current, valid, unrestricted Nurse Practitioner license from the State of Wisconsin; current board certification or current board certification from ANCC, ANA, or AANP for NP must be achieved within 18 months of hire; and current unrestricted DEA.
  • BLS required
  • Minimum of three years clinical experience required.  Chronic Disease Management, Palliative Care, and/or Hospice experience preferred.
  • Achieves palliative care board certification within five years of employment as Palliative Nurse Practitioner or the applicant must be functioning as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) with hospice and palliative advanced nursing practice experience of 500 hours in the most recent 12 months or 1000 in the most recent 24 months.

  • Assist housekeeper and laundry with preventive maintenance (PM) in all areas of the hospital.
  • Performs minor repairs and preventative maintenance to hospital equipment and to the facility's interior and exterior.
  • Uses safe methods when handling chemicals and cleaning solutions in daily work practice.

Educational Qualifications:

  • High school diploma or general education degree (GED).
  • Experience and/or training in housekeeping, laundry, painting, hand tools, preventative maintenance, equipment, buildings, and grounds.
