BUSINESS INTELLIGENCE - INFORMATICS NURSE

It’s an exciting time to join our growing organization and our internal Business Intelligence team as the Informatics Nurse for the providers, Hospice and Home Care, and the clinic.  This new position is benefited and full-time at 80 hours per pay period, day shift, Monday through Friday.

The Informatics Nurse for Black River Memorial Hospital (BRMH) integrates nursing, computer science and information management to manage data, information, and knowledge within the clinical departments.  They are responsible for specific tasks associated with the implementation, training, and maintenance of clinical application systems within BRMH.  They also serve as a communication intermediary with other departmental users during development, installation and end-user dissemination.

Essential Duties:

  • Works closely with the project sponsor, project leader, and analyst throughout clinical system implementations.
  • Ensures clinical needs are evaluated and validated when implementation plans are developed by collecting input from end-users.
  • Is proficient in the operations of the suite of clinical applications as new systems are installed and existing systems are upgraded.
  • Manages current state workflows for clinical departments.
  • Assess the implications on nursing operations and processes of patient care delivery when assisting in the resolution of departmental issues.

Educational Qualification:

  • Graduate of an accredited school of nursing.  BSN required.
  • Licensed as a registered nurse in the state of Wisconsin
  • 5 years of experience in a clinic setting.  2 years of experience with clinical information systems.
  • Management experience helpful.  Computer knowledge required.
  • Ability to read, write, type, and speak English.  Has a working knowledge of the nursing process.  Communicates, interprets and critically thinks using nursing perspective.  Operates a computer and uses critical thinking skills to troubleshoot computer applications.

CLINIC MEDICAL DIRECTOR

Black River Memorial Hospital is excited to announce an opportunity to join the Black River Memorial Hospital Primary Care and Behavioral Health Clinic team as the Clinic Medical Director.  This position is full-time, exempt, 80 hours per pay period with a generous benefits package. 

The Clinic Medical Director provides oversight and direction of the daily operations of the Black River Memorial Hospital Primary Care and Behavioral Health Clinic. In addition, it works directly with the Clinic Director to ensure the effective management of clinic processes and ancillary staff.  It directly supervises the Clinic Providers to assure safe and best practice operation of the care delivery processes and assist in identifying opportunities for enhanced efficiencies in the delivery of care, productivity, performance improvement and standardization of process.  It also works as a patient care provider evaluating and treating patients.

Essential Duties:

  • Provide appropriate medical services (assess, treat, admit, transfer or discharge) to all patients who seek care in the Black River Memorial Hospital Primary Care and Behavioral Health Clinic. Perform the duties of a primary care physician in an outpatient medical clinic.
    • Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury;
    • Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients' condition;
    • Follows all existing referral patterns with respect to the referral of patients for specialist care;
    • Uses currently accepted methods and practices of medicine, adheres to applicable standards of care, medical ethics, policies, and protocols.
  • Oversee the day-to-day management of department medical provider staff and schedules work to facilitate effective and efficient department operations. Direct and coordinate activities of nurses, students, assistants, specialists, therapists, and other medical staff.   Select and hire quality employees.
  • Investigate and take appropriate timely action(s) to resolve customer concerns, complaints, and problems. Utilize the established customer service recovery process.
  • Provide leadership, coaching, and development for providers. Completes self, peer, and staff annual performance appraisals.  Follows hospital Standards of Conduct and holds staff accountable for meeting the Standards of Conduct.

Education Qualifications:

  • Doctorate Required, Field of Study: Family Medicine
  • Master's Degree in Public Health or Medical/Healthcare Administration preferred.
  • Minimum of 5 years of experience in clinical practice required.
  • Minimum of 2 years of experience in a leadership role within a medical practice or complex physician organization is required, 5 years is preferred. Leadership experience should include direct management of providers.
  • Proficient in Microsoft Office.
  • Medical License in the State of Wisconsin.
  • Board certification in Family Medicine is required.
  • Demonstrated ability to lead by example, effectively communicates with diverse groups, promote shared accountability, and facilitate change with a clear focus on results.

EMERGENCY DEPARTMENT PARAMEDIC

Black River Memorial Hospital (BRMH) has an immediate opening in our Emergency Department for an experienced Critical Care Paramedic.  This benefited position is part-time, 48 hours per pay period, work 12 hours/on-call for 12 hours, two days per week. 

As a paramedic at BRMH, you will provide paramedic-level emergency medical care for patients of all ages.  This position functions according to Wisconsin Paramedic Curriculum and functions under the direction of the emergency department nurse or provider.

Essential Duties:

  • Maintains up to date knowledge and cares for patients in accordance with department policies, procedures, protocols and scope of practice/job description.
  • Performs various medical procedures under the direction of the Emergency Department nurse or provider.

HOME HEALTH AIDE

Black River Memorial Hospital has an immediate opening for a Home Health Aide in our Homecare/Hospice Department.  This is a part-time position, working 64 hours per pay period and is fully benefitted.   The Home Health Aide provides and/or assists with personal care service for Hospice patients and families.

Education Requirements:

  • Minimum Education: Basic reading and comprehension skills.
  • Preferred Education: High school diploma or equivalent.
  • Current certification by the WI State Board of Nursing and meets the personnel, education and training requirements of CFR 484.4 for home health aides.
  • Experience with Hospice/End of Life Care preferred (for hospice program).

HOMECARE/HOSPICE REGISTERED NURSE

  • Experience with Hospice and/or Homecare preferred.

Qualifications:

NUTRITION SERVICES ASSISTANT

  • Assists in preparing and portioning food for patients using room service or scheduled meals, contracted food services, employees and hospital special functions following established policies and procedures.
  • Accurately and appropriately transports scheduled patient meal cart to the patient floor and delivers patient’s room service trays or late trays directly to the patient’s room.  Collects soiled trays from patient rooms and document the amount of food consumed by the patient.
  • Performs various cleaning duties per department protocol.  Keeps work area clean and in a sanitary condition at all times.

OCCUPATIONAL THERAPIST

SOCIAL WORKER (2)

Now is your chance to become a member of the Patient and Family Services (P&FS) team at Black River Memorial Hospital (BRMH) as a Social Worker.  BRMH has 2 part-time, 48 hours per pay period openings that also include our generous benefits package.  In addition, as part of the P&FS Team, you will participate in the on-call rotation of one weekend a month and one night a week and assist to fill on-call gaps as they periodically arise.

This position plays an important role with our patients and works as part of the total health care team to help meet the social, economic, emotional and spiritual needs of patients and their families during hospitalization or times of crisis. 

Essential Duties:

  • Maintains standards of Social Work excellence in accordance with standards set forth by the Wisconsin Department of Health Services, National Association of Social Work, the Joint Commission, and the policies and procedures of the hospital.
  • Receives consults from a multidisciplinary team based on admission assessment, patient/family requests, physicians orders, multidisciplinary discharge planning meetings and outside hospitals. Identifies patients in need of discharge planning during the multidisciplinary meeting.
  • Participates in the multidisciplinary discharge planning process. Incorporates a multidisciplinary approach to include the patient, hospital staff, and patient’s family/significant others to develop and implement a plan to meet the patient’s needs.  Facilitates multidisciplinary discharge planning meetings and care conferences.
  • Documents social work interventions consistent with documentation guidelines.
  • Prepares, maintains and keeps current departmental records, reports, statistics, memos and other documents as required.

Educational Qualifications:

  • Bachelor’s degree from a four-year college or university in social work or related field.
  • 1-2 years related experience in a health care setting preferred.
  • Current social work certification from the State of Wisconsin.
