Complete Your Power of Attorney For Healthcare Document


Black River Memorial Hospital (BRMH) is announcing Black River Falls Mayor, Jay Eddy has proclaimed April 16th as Healthcare Decisions Day which highlights the importance of completing a Power of Attorney for Healthcare document, also known as an advance directive.

An advance directive allows you to choose someone to help you with your healthcare decisions if you are unable to do it yourself. Michelle Goetting, BRMH Patient and Family Services Coordinator said, “One of the most important things you can do to make sure you remain in control of directing your health care is to complete a Power of Attorney for Healthcare document. If you become sick or injured and are not able to communicate your wishes, this document lets your care team and loved ones know what you want.” She added, “It’s a win-win for everyone when your choices are known ahead of time.”

To complete a Power of Attorney for Healthcare document, forms are available at under the community resources tab or call 715-284-5361 and ask to speak with a social worker.
